Job summary

Guardian Angel Home Care is seeking compassionate Physical Therapist Assistants to join our Home Health team. The PTA works under a licensed Physical Therapist to deliver medically prescribed therapy in patients' homes, helping improve mobility, strength, independence, and quality of life.

This role offers field-based, in-home patient care across service areas nationwide, with opportunities to grow within a rapidly expanding home health organization.

Qualifications

• Graduate of a PTA program accredited by APTA/AMA.
• Current PTA license in the state of employment.
• Minimum one year of clinical experience preferred.
• Home Health experience preferred but not required.

Responsibilities

• Conduct prescribed therapy programs in patients' homes.
• Record and report patient progress and response to treatment.
• Collaborate with supervising Physical Therapists and care teams.
• Prepare and maintain clinical documentation.
• Follow infection control standards and agency policies.
