Job summary

VNS Health seeks a Physical Therapist Assistant to provide PT services under supervision, traveling to patients’ homes and facilities in New York. You will implement the PTA plan of care, monitor progress, and communicate with supervising PT to adjust treatments as needed.

Minimum two years of clinical patient care experience and NY PTA certification are required; an associate degree from a NY-approved program is essential.

Qualifications

• Two years of clinical patient care experience required.
• New York State Certification as Physical Therapist Assistant required.

Responsibilities

• Follow established PT plan of care and provide PTA services.
• Monitor patient progress with supervising therapist and document care.
• Co-visit with PT on initial PTA visits; PT re-assesses every 6 visits or 30 days.
• Manage caseload and deliver direct PT services as directed.
• Travel to patients' homes or facilities to provide care.
• Assist patients with activities of daily living and use of durable medical equipment.
• Maintain infection control and proper documentation; participate in team meetings.
