Job summary

Personal Healthcare Management LLC is seeking a Physical Therapy Assistant (PTA) in New York. The role requires an NYS PTA license and an associate’s degree in physical therapy, with geriatric experience preferred.

A valid driver’s license and ability to work with residents, staff, and families are essential. The PTA will provide plan-of-care services under a licensed PT, document progress, and assist with daily activity schedules.

Qualifications

• Must possess a current unencumbered NYS license as a Physical Therapy Assistant (PTA).
• Graduate with an associate’s degree in physical therapy from an accredited program.
• Geriatric experience preferred to support residents.
• Valid driver’s license and reliable transportation required.
• Proficient with essential computer applications and EHR software.
• Ability to communicate professionally with staff, residents, and families.
• Ability to work autonomously and as part of a team.

Responsibilities

• Provides PTA services and modalities under supervision of a licensed Physical Therapist.
• Communicates effectively with residents, families, and staff.
• Relates information about residents’ conditions to team members.
• Trains and supervises subordinate staff as needed.
• Documents progress toward therapy goals and updates notes.
• Completes daily schedules and progress notes per facility policy.
• Educates residents, staff and families as directed.
• Complies with rehab regulations and facility policies.
• Uses clinical software per procedures and orders necessary equipment.
• Maintains confidentiality and respects resident rights.
• Reports suspected resident abuse or neglect promptly.
