Job summary

Freedom at Home is seeking a compassionate Physical Therapy Assistant (PTA) to deliver skilled therapy in patients' homes under the supervision of a licensed Physical Therapist. You will implement treatment plans and support mobility, balance, and independence.

The role offers full-time, part-time, and PRN opportunities with paid mileage, flexible scheduling, and strong leadership support to help clinicians grow professionally and make a meaningful difference daily.

Qualifications

• Graduate of an accredited Physical Therapist Assistant (PTA) program.
• Current Physical Therapist Assistant license in the state of practice.
• Current CPR certification.
• Minimum of two years of PTA experience preferred.
• Home Health experience is a plus but not required.
• Valid driver's license, reliable transportation, and auto insurance

Responsibilities

• Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist
• Carry out individualized treatment plans designed to improve patients' mobility, strength, balance, function, and overall quality of life
• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and use of assistive devices
• Monitor patient progress and communicate changes to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Complete accurate, timely clinical documentation
• Participate in case conferences, team meetings, and quality improvement initiatives
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to provide coordinated patient care
• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes

Freedom at Home, a Combat and Service Disabled Veteran owned and operated Small Business, is seeking a compassionate and dedicated Physical Therapy Assistant (PTA) to join our team of highly valued clinicians. As a PTA Home Health, you will provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist to patients in the comfort of their own homes.
