Job summary

Carter Nursing & Rehab is seeking a Physical Therapy Assistant (PTA) to join our care team in a full-time role. You will support the delivery of rehabilitative services under the supervision of a licensed Physical Therapist, focusing on mobility, pain reduction, and functional independence for residents.

Ideal candidates have an Associate’s degree, current state licensure, 1–2 years in geriatric or skilled nursing settings, and familiarity with PDPM documentation.

Qualifications

• Associate’s degree from an accredited Physical Therapist Assistant program.
• Current state licensure as a Physical Therapist Assistant.
• 1-2 years of experience in a skilled nursing facility or geriatric care setting.
• Familiarity with PDPM and SNF therapy documentation standards is a plus.

Responsibilities

• Deliver physical therapy interventions as outlined in the treatment plan developed by the supervising Physical Therapist.
• Assist residents with therapeutic exercises, gait training, and functional mobility activities to improve strength, balance, and independence.
• Monitor resident responses during treatment sessions and communicate progress, concerns, or changes to the Physical Therapist.
• Support resident and family education under the direction of the Physical Therapist to promote safe and effective carryover of therapy strategies.
• Maintain a safe, supportive, and engaging therapeutic environment that encourages resident participation and comfort.
• Participate in interdisciplinary team meetings and contribute to care planning discussions to ensure coordinated, resident-centered care.
