Creekwood Nursing and Rehabilitation is seeking a licensed Physical Therapy Assistant to provide PT services under the direction of a licensed PT. You will support patient care plans, supervise rehab techs, and contribute to high-quality therapy within a team-driven facility.

Responsibilities

• Reports to the Director of Rehabilitation and Physical Therapist.
• Assist in screening and providing input to initial evaluations as appropriate.
• Implement treatment for all assigned patients per the plan of care.
• Maintain accurate documentation and billing per guidelines.
• Maintain effective patient-centered interactions with the team and staff.
• Provide supervision to rehabilitation technicians during group and individual treatment.
• Integrate physical therapy services into care plans.
• Participate in utilization review, case management, and meetings.
• Maintain professional relationships with healthcare professionals.
• Refer to other rehabilitation team members when appropriate.
• Communicate patient progress and issues to supervising PT.
• Adhere to safety, infection control, and privacy policies.
• Pursue continuing education and professional development.
