Job summary

Magnolia Creek Nursing and Rehabilitation is seeking a dedicated Physical Therapist Assistant to provide therapy services under the direction of a licensed PT. You will work with residents to improve strength, mobility, and overall confidence within a supportive interdisciplinary team.

The role emphasizes quality patient care, adherence to plan of care, supervision of rehab techs, and participation in care conferences.

Qualifications

Responsibilities

• Reports to the Director of Rehabilitation and Physical Therapist.
• Assist in screening and input to initial evaluations.
• Implement treatment per the established plan of care.
• Comply with documentation and billing guidelines.
• Supervise rehabilitation technicians during group/individual treatment.
• Integrate PT services into facility and interdisciplinary plans.
• Participate in care conferences and team meetings.
• Maintain effective relationships with healthcare professionals.
