Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Orange, NJ. The PRN position supports the Physical Therapist through direct patient care, screenings, and treatment planning.

The role emphasizes collaboration with the care team, documentation in NetHealth EMR, and ongoing professional development to maintain licensure. A PTA license in the state and an associate degree are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and in identifying goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatment as outlined in the treatment plan.
• Assist in selecting appropriate physical therapy techniques/media and adapt as indicated.
• Monitor residents' response and modify treatment with the Physical Therapist as needed.
• Assist in establishing and modifying resident schedules with the Physical Therapist and team.
• Adhere to resident schedules and modify as appropriate for treatment regimen.
• Train residents, family, and staff in use of equipment and therapies.
• Participate in resident, family, and staff education.
• Maintain sound judgment in evaluating and following up on therapy programs.
• Report to work on time and coordinate schedules for productivity.
• Assist with Quality Assurance activities and CMS documentation in NetHealth EMR.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license
