Job summary

Shore Acres Care Center in St. Petersburg, FL is seeking a passionate Physical Therapy Assistant (PTA) to join our rehabilitation team.

The role focuses on delivering high-quality patient care for short-term rehabilitation and long-term care residents in a collaborative, patient-centered environment. The ideal candidate demonstrates strong clinical judgment, excellent communication, and thorough documentation.

Qualifications

• Current Florida Physical Therapy Assistant license in good standing.
• Graduate of an accredited Physical Therapy Assistant program.
• Experience in a skilled nursing facility, post-acute rehabilitation, or long-term care setting preferred.
• Knowledge of PDPM, Medicare Part A and Part B, and skilled therapy documentation requirements.

Responsibilities

• Provide comprehensive physical therapy treatment based off POC and deliver evidence-based interventions to maximize functional independence and quality of life.
• Demonstrate knowledge of the PDPM, Medicare guidelines, skilled therapy requirements, and documentation practices.
• Develop and support long-term care programming, including fall prevention and functional maintenance.
• Collaborate with therapists and interdisciplinary team to coordinate care.
• Maintain accurate, timely documentation supporting medical necessity and progress.
• Participate in interdisciplinary meetings and care conferences.
• Monitor patient progress and adjust treatment plans with the PT as appropriate.
• Communicate effectively with residents, families, and facility leadership.
