Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. Full time with a $5,000 relocation or sign-on bonus, plus flexible scheduling as options.

Responsibilities include providing therapy under the PT, conducting screenings, aiding in evaluation and treatment planning, and ensuring proper documentation in NetHealth EMR. Requires an associate degree and an active PTA license.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under supervision.
• Conduct screenings to determine need for intervention.
• Assist in evaluations and treatment planning with the Physical Therapist.
• Monitor residents' response to intervention and modify as needed.
• Assist with scheduling and coordination of resident therapy sessions.
• Train residents and staff in use of equipment and therapies.
