Job summary

Beaumont Home Health & Hospice in Detroit, MI seeks a dedicated Physical Therapist Assistant to provide home-based skilled therapy as directed by physicians and PTs. You will work with an interdisciplinary team to advance patient recovery in diverse Detroit-area neighborhoods.

The role requires an active PTA license in the state, and a minimum of one year of PTA experience in acute care; home care experience is preferred.

Qualifications

• Licensed Physical Therapist Assistant in the state of employment.
• Minimum one year of PTA experience in an acute care setting.
• Home care experience is preferred.

Responsibilities

• Plan of Care: provide skilled PTA services as ordered by the physician and monitor patient status.
• Patient outcomes: assist in relieving pain, restoring function, and maintaining performance.
• Minimize patient risk through assessment and intervention with PT guidance.
• Family educator/advocate: educate patient and family on disease, management, and device use.
• Policies: complete documentation per agency protocol and Medicare guidelines.
• Rules and regulations: follow agency policies and communicate schedule changes.
• Operations: participate in in-service training and mandatory meetings.
