Job summary

The Cane Ridge Post-Acute and Rehabilitation Center in Antioch, TN is seeking a dedicated Physical Therapist Assistant to support residents under the supervision of a Physical Therapist. You will implement treatment plans, monitor responses, educate families, and document services to aid mobility and independence.

Qualifications

Graduate of an accredited Physical Therapist Assistant program.
Current, unencumbered PTA license in Tennessee.
Long-term Care/skilled Nursing Experience preferred.
Strong communication, organization, and teamwork skills.

Responsibilities

Implement treatment plans established by the Physical Therapist, using appropriate therapeutic exercises and activities.
Monitor resident responses to treatment and report changes to the PT.
Assist with resident and family education regarding home exercise programs and safety.
Complete timely and accurate documentation of services provided.
Collaborate with the interdisciplinary team to support resident goals and discharge planning.
