Job summary

Cane Ridge Post-Acute and Rehabilitation Center in Antioch, TN seeks a Physical Therapist to evaluate, plan, and implement individualized therapy aimed at improving mobility and quality of life for residents.

Responsibilities include conducting evaluations, developing treatment plans, supervising PTAs, documenting in the medical record, collaborating with the care team, and educating residents and families on therapy programs and safety. TN license required; long-term care experience preferred.

Qualifications

• Bachelor's degree or higher in physical therapy from an accredited program.
• Current, unencumbered PT license in Tennessee.

Responsibilities

• Perform physical therapy evaluations and re-evaluations per professional standards and payer requirements.
• Develop and implement treatment plans tailored to resident goals and functional abilities.
• Supervise and direct PTAs in treatment plan implementation where applicable.
• Complete timely, accurate documentation in the medical record and therapy system.
• Collaborate with the interdisciplinary team on care planning, discharge recommendations, and equipment needs.
• Educate residents, families, and staff on therapy programs, safety, and use of assistive devices.

Position Summary
