Job summary

Trevida Healthcare is seeking a dedicated Physical Therapist Assistant (PTA) to work under the supervision of a licensed Physical Therapist. You will design and implement individualized treatment plans to promote independence, mobility, and well-being for our residents.

The role emphasizes collaboration with the rehabilitation team, proper documentation, and ongoing professional development within a supportive environment.

Qualifications

• Associate degree required.
• CAPTE-accredited PTA program required.
• Current Ohio PTA license or certification required.
• Experience in long-term care or rehabilitation preferred.
• Strong communication and interpersonal skills required.
• Ability to lift, transfer and move patients as needed.

Responsibilities

• Assist licensed Physical Therapists with patient assessments and treatment plan development.
• Conduct physical therapy treatments including exercises and modalities as prescribed.
• Monitor and document patient progress and report changes to the Physical Therapist.
• Maintain accurate and thorough patient records in accordance with facility policies.
• Collaborate with the rehabilitation team to ensure comprehensive care and effective communication.
• Participate in staff training and education to stay current with best practices.
