Job summary

Career Links is seeking an energetic Physical Therapist Assistant (PTA) to join our skilled nursing team, working under the supervision of a licensed Physical Therapist to provide evidence-based interventions designed to maximize residents’ mobility, function, and independence. The PTA will implement treatment plans, assist residents with exercises and mobility activities, and monitor progress.

Qualifications

• Graduate of an accredited PTA program.
• Current State PTA license or eligibility.
• Strong interpersonal, organizational, and communication skills.

Responsibilities

• Implement treatment plans under the supervision of the licensed Physical Therapist.
• Assist residents with exercises, gait training, and functional mobility activities.
• Monitor and document residents’ progress toward treatment goals.
• Communicate effectively with the therapy team, nursing staff, and families regarding patient status and care needs.
• Ensure documentation meets facility, state, and payer requirements.
• Participate in team meetings, discharge planning, and interdisciplinary collaboration.
• Support rehabilitation goals of the SNF with professionalism and compassion.
