Qualifications

• Associates degree from an accredited school of Physical Therapy.
• Must possess and maintain valid CPR certification.
• Must have a valid driver's license and be able to travel to all locations.

Responsibilities

• Provide patient care and document treatment per physician orders and policy.
• Document interventions and treatment priorities.
• Educate and counsel patients and families.
• Present patient information to supervising therapist and keep team informed.
• Deliver skilled treatment according to patient needs and payer parameters.
• Collaborate with team to plan and intervene.
• Complete and submit daily notes and forms on time.
• Meet productivity standards and attend agency meetings.
