Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Florida PTA license in good standing.
• Current CPR certification.
• Valid Florida driver's license and reliable transportation.

Responsibilities

• Provide physical therapy treatments as outlined in the patient's Plan of Care.
• Monitor patient progress toward established goals and document accordingly.
• Educate patients and families on exercises, mobility techniques, and fall prevention.
• Communicate changes in patient condition to the supervising Physical Therapist.
• Maintain timely, accurate, and compliant clinical documentation.
• Collaborate with nurses, therapists, physicians, and office staff to ensure coordinated care.
• Follow Medicare, state, and company regulations and standards.
• Promote patient safety and professional conduct during home visits.

Position Summary

The Physical Therapist Assistant (PTA) works under the direction and supervision of a licensed Physical Therapist to provide high-quality rehabilitative care in patients' homes. This role focuses on helping patients improve mobility, strength, balance, safety, and overall functional independence while maintaining accurate documentation and collaborating with the interdisciplinary care team.

• Provide physical therapy treatments as outlined in the patient's Plan of Care.

• Excellent communication and interpersonal skills.
• Ability to work independently while managing a flexible schedule.
• Home health experience preferred, but not required.
