Pinnacle Home Care is Florida's largest Medicare-certified home health provider and seeks a Full-Time Physical Therapist Assistant (PTA) to join our award-winning team. You will work in Southern Citrus County and surrounding communities to deliver high-quality, one-on-one care in patients' homes.

Responsibilities include evaluating care plans, documenting progress, performing therapeutic exercises, and educating patients and families.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Licensed PTA in Florida with current BLS certification.
• Reliable transportation with valid driver’s license and insurance.
• One year of home healthcare experience preferred (new grads welcome).
• Experience with electronic medical records is a plus.
• Strong physical stamina and excellent communication skills.

• Document clinical and daily progress notes for PT review and submission.
• Perform therapeutic exercises and assist patients with self-help devices.
• Instruct patients and families on therapy exercises alongside the PT.
• Monitor and report adverse drug reactions or ineffective therapies to the PT.
• Observe, document, and report clinical findings to the PT and physician as needed.
• Coordinate with PT and nursing staff on discharge plans and summaries.
• Educate health team members and families to support patient care.
• Maintain accurate documentation and comply with agency policies.
