Job summary

CareMed Health Solutions seeks a Wound Care Nurse Practitioner to deliver wound care in long-term care facilities. You will manage treatment plans, work with multidisciplinary teams, and coordinate with primary care providers to optimize patient outcomes.

The role requires KY NP licensure, board certification, and experience with wound management and documentation in an EMR system. This is a facility-based position with opportunities for professional growth.

Qualifications

• Active, unrestricted Nurse Practitioner license in Kentucky.
• Board certification in a relevant specialty.
• DEA licensure is optional but preferred for prescribing controlled substances.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances in KY as permitted by law.
• Order, review, and follow up on treatments and diagnostics.
• Communicate with nursing home staff about changes in patient condition and care plans.
• Collaborate with the primary care physician to coordinate care.

Description

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

Wound Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIA P
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care
