Job summary

Fellowship Community in Whitehall, PA, seeks a PT Social Worker to support residents' physical, emotional, and psychosocial well-being. You will partner with the SW/CM to keep transitions smooth, ensure clear communication, and uphold resident rights daily.

Role focuses on discharge planning, coordinating among residents, families, and the interdisciplinary team, and advocating for resident rights in line with Fellowship standards. Flexible 25-hour week on-site.

Qualifications

  • Bachelor’s degree in Social Work required.
  • Minimum two years’ geriatric care experience.
  • Strong interpersonal, public relations and organizational skills.
  • Ability to counsel individuals in crisis and know senior resources.
  • Intermediate computer skills and MDS familiarity.
  • Understanding of Medicare/MA/private pay basics.

Responsibilities

Your Mission

As a PT Social Worker, you’ll partner closely with the SW/CM to drive strong resident outcomes by supporting each person’s physical, emotional, and psychosocial well-being. This role exists to keep transitions smooth, communication clear, and resident rights front and center—every single day.

Key responsibilities include assisting with discharge planning, coordinating communication between residents, families/representatives, and the interdisciplinary team, and advocating for resident rights in line with Fellowship standards.

Required qualifications/skills:

  • Bachelor’s degree in Social Work
  • Minimum two years’ experience in geriatric care
  • Excellent interpersonal/public relations skills; strong organization
  • Ability to counsel individuals in crisis; knowledge of senior community resources
  • Intermediate computer skills; working knowledge of the MDS
  • Understanding of Medicare/MA/managed care/private pay basics; medical terminology
