Job summary

Fellowship Community in Pennsylvania is seeking a part-time PT Social Worker to partner with the SW/CM and drive resident well-being. You will support discharge planning, coordinate communication among residents, families, and the interdisciplinary team, and advocate for resident rights in line with Fellowship standards.

The role requires a Bachelor’s in Social Work, at least two years in geriatric care, strong interpersonal skills, crisis counseling abilities, and working knowledge of the MDS

Qualifications

  • Bachelor’s degree in Social Work required.
  • Minimum two years’ experience in geriatric care.
  • Excellent interpersonal/public relations skills; strong organization.
  • Ability to counsel individuals in crisis; knowledge of senior community resources.
  • Intermediate computer skills; working knowledge of the MDS.
  • Understanding of Medicare/MA/managed care/private pay basics; medical terminology.

Your Mission

As a PT Social Worker, you’ll partner closely with the SW/CM to drive strong resident outcomes by supporting each person’s physical, emotional, and psychosocial well‑being. This role exists to keep transitions smooth, communication clear, and resident rights front and center—every single day. Key responsibilities include assisting with discharge planning, coordinating communication between residents, families/representatives, and the interdisciplinary team, and advocating for resident rights in line with Fellowship standards.
