Stern Therapy Consultants is seeking a Physical Therapist (PT) for a PRN opportunity at Nellas Autumn Lake in Elkins, WV. The role offers a competitive $55–$60/hour rate and a collaborative rehabilitation team focused on improving mobility, strength, balance, and independence for residents.

Responsibilities include evaluations, individualized plans of care, and ongoing adjustment of treatment to patient response, with emphasis on safe mobility and caregiver education.

Qualifications

• Active West Virginia Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and teamwork skills.

Responsibilities

• Perform PT evaluations and develop individualized plans of care.
• Provide treatment addressing strength, balance, gait, transfers, endurance, and mobility.
• Establish functional goals and monitor resident progress.
• Modify treatment plans based on clinical needs and response to therapy.
• Educate residents and caregivers on exercises and safe mobility techniques.
• Maintain clear and timely clinical documentation.
• Coordinate care with PTAs, nursing staff, and other rehabilitation professionals.
