Job summary

Autumn Lake at Greenfield in Milwaukee, WI is seeking a PRN Physical Therapist (PT) to provide individualized rehabilitation services in a collaborative skilled nursing and post-acute care setting.

Responsibilities include evaluations, planning, and interventions to improve strength, balance, gait, and mobility; documentation; and teamwork with PTAs and nurses. Hourly pay is $55-$65.

Qualifications

• Active Wisconsin Physical Therapist license required.
• Strong clinical assessment and communication skills.
• Experience in skilled nursing or post-acute care is a plus.
