Autumn Lake at Greenfield is seeking a PRN Physical Therapist (PT) to provide individualized rehabilitation services in a collaborative skilled nursing and post-acute care setting in Milwaukee, WI.

The role involves performing PT evaluations, establishing plans of care, and delivering interventions to improve strength, balance, gait, and mobility, while closely monitoring outcomes and coordinating with other rehabilitation professionals.

Qualifications

• Active Wisconsin Physical Therapist license required.
• Strong clinical assessment and communication skills.
• Skilled nursing or post-acute care experience is a plus.

Responsibilities

• Perform physical therapy evaluations and establish individualized plans of care.
• Provide skilled interventions focused on strength, balance, gait, mobility, and functional performance.
• Monitor resident outcomes and modify treatment plans as appropriate.
• Educate residents and caregivers on mobility and safety strategies.
• Work closely with PTAs, nursing, and other rehabilitation professionals.
• Maintain timely clinical documentation.
