Continuum Therapy Partners in Willmar, MN is seeking a Physical Therapist for PRN coverage. You will join a team that values your clinical voice and flexible scheduling.

The role requires a Doctor of Physical Therapy (DPT), active Minnesota license and NPTE, with a preference for 1 year of PT experience. You will evaluate, develop and implement personalized treatment plans collaborating with other disciplines.

Qualifications

• DPT degree required.
• Current, unencumbered Minnesota PT license; must pass NPTE.
• 1 year of PT experience preferred.

Responsibilities

• Documents patient progress and adjusts plans of care based on outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
