Job summary

Elderwood is seeking a high-quality, engaging Physical Therapist to be on the front lines, making a difference in the lives of our residents and their families.

Physical Therapy team members administer skilled therapies, ensuring top‑quality care. State licensure is required, and two years of PT experience is preferred. This role may involve supervision and collaboration with other care staff in a resident-centered environment.

Qualifications

• Graduation from an approved school of physical therapy is required.
• State license to practice physical therapy is required.
• Two years of experience in physical therapy preferred.
• Must be able to direct others as needed.
• English reading/writing/speaking proficiency at an intermediate level or higher.

Responsibilities

• Initiates skilled physical therapy treatments per physician orders and maintains contact with the physician.
• Maintains evaluations, progress notes and discharge summaries for the therapy program.
• Collaborates with other health care staff to support rehabilitation or maintenance of function.
• Participates in meetings and covers for the Director of Physical Therapy during absences.

Physical Therapist Position Overview

Physical Therapy team members assist with ensuring the health and well-being of individuals by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments ensuring that the highest degree of quality care is maintained at all times.

State specific Physical Therapy license is required.

Responsibilities

• Initiates skilled physical therapy treatments in accordance with physician's orders; maintains required contact with physician.
• Maintains written evaluations, progress notes and discharge summaries for individuals in physical therapy program.
• Confers with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.
• Provides input for meetings and attends during absences of the Director Physical Therapy.
• Maintains statistical records of individuals’ treatments.
• Assists with in-service training programs upon request.
• Records and updates individual's plan of care in the Inter-disciplinary Kardex.
• Submits requisition for supplies needed in the treatment to Director Physical Therapy.
• Assumes all duties of the Director Physical Therapy during absences including supervision of Physical Therapy Aides and responsibility for all necessary documentation.

Qualifications

• State license to practice physical therapy required.
• Two years' experience in physical therapy preferred.
• Ability to direct others necessary.
• This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
