Job summary

Adoration Health seeks a licensed Physical Therapist for Home Health in Dublin, GA. You will provide 1:1 patient care, conduct evaluations, and implement treatment plans in a supportive home setting.

Ideal candidates have a PT degree, a valid GA license, CPR certification, and at least 2 years of experience. The role offers full-time options with potential sign-on or relocation assistance through Adoration/Home Health services.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New grads may be considered in select markets.
• At least 2 years of PT experience; hospice/community experience preferred.
• Valid license in state of practice.
• Current CPR certification.

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders.
• Develop and adjust treatment plans to achieve independence.
• Maintain communication with treatment team, patient and family.
• Ensure thorough documentation for proper billing.
• Supervise PTAs and PT aides as applicable.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license from the state of practice
• Possess and maintains current CPR certification
