Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is hiring a Home Health Physical Therapist in Brunswick, GA. Join a compassionate team delivering patient‑centered care in the home with a focus on independence and quality outcomes.

In this role you will conduct evaluations, develop treatment plans, and collaborate with families and the care team. A CAPTE‑accredited PT program, state licensure, and CPR are required; experience in adult home health is preferred.

Qualifications

• CAPTE-approved Physical Therapist education program.
• Current Physical Therapist licensure in state and CPR certification.
• Two years of experience in adult physical therapy; home health preferred.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment recommendations.
• Set measurable goals and develop treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient and family.
• Maintain thorough documentation for accurate billing.
• Supervise assistants and aides in direct patient care as applicable.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE
• Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
• Two (2) years of experience in adult physical therapy, home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist
