Adoration Health is seeking a Physical Therapist for home health in Vidalia, GA. You will provide one-on-one patient care, assess and treat patients according to physician orders, and collaborate with families and the care team to improve function and independence.

The position is full-time in Vidalia with a $10K sign-on bonus for external candidates, and comprehensive benefits supporting professional growth in a compassionate home-health setting.

Formación

• Bachelor's degree in Physical Therapy from an accredited program.

Responsabilidades

• Perform PT evaluation and treatment based on physician orders, including adaptive equipment recommendations.
• Establish measurable goals and develop plans to improve independence.
• Maintain communication with the treatment team, patient, and family as applicable.
• Maintain thorough, timely documentation for proper billing.
• Supervise PTAs and PT Aides in patient care per state practice act.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
