Field PT Physical Therapist for Medicare Certified Home Health - Per Diem W2 Employee (PRN)

We are seeking an experienced Home Health Physical Therapist to serve our Central-North Pinellas territory.

JOB SUMMARY:

A Registered Physical Therapist (PT) administers physical therapy to patients in their homes on an intermittent basis, following physician orders and care plans under the supervision of the Director of Clinical Services/Clinical Manager.

QUALIFICATIONS:

• Graduate from an accredited physical therapist program approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or equivalent.
• Current Florida license.
• Two years’ experience preferred.
• Aligned with the agency's philosophy and goals.
• Ability to exercise initiative and independent judgment.
• Current CPR certification, TB test, health statement/physical clearance, and eligible for Level 2 background check.

RESPONSIBILITIES:

• Adhere to established policies and procedures.
• Provide physician-prescribed physical therapy.
• Improve or minimize residual physical disabilities.
• Assist patients to regain optimal and productive levels within their capabilities.
• Participate in patient care planning with home care team members.
• Supervise personnel as required.
• Conduct initial assessments, develop and modify plans of care, and perform reassessments every 30 days.
• Perform skilled procedures as ordered by a physician.
• Consult with physicians regarding treatment changes.
• Instruct patients and families on home programs and daily activities.
• Prepare clinical and progress notes.
• Assist physicians in evaluating patient function levels.
• Develop and revise the plan of care as needed.
• Coordinate with family and agency personnel.
• Complete and submit OASIS assessments, reassessments, transfers, care resumptions, discharges, and significant condition changes within agency-defined timeframes.
