Manatee Health at Home by BAYADA is seeking compassionate and dedicated Per Diem Physical Therapists to join our team. This role services adult and geriatric clients on a per visit basis across territories including RIVERVIEW, VALRICO, BRANDON, and SOUTH TAMPA, FL.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Implement integumentary protective interventions to prevent wound formation and promote wound healing using evidence-based dressings and topical agents.
4. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
5. Assess and revise the care plan regularly, collaborating with nursing staff as part of the multidisciplinary team.
6. Educate clients, family members, or caregivers in rehabilitative activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.

Qualifications for a PT - Physical Therapist include:

• Current FL Physical Therapist - PT license.
• Graduation from an American Physical Therapy Association-approved program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; prior EMR experience preferred.
