Job summary

Life Enriching Communities is expanding its in-house physical therapy team at Wesley Ridge in Reynoldsburg, OH. This part-time, on-site position focuses on delivering personalized rehabilitation and helping residents regain independence.

You will conduct initial exams, design treatment plans, document progress, and collaborate with medical staff. A graduate of an accredited PT program with at least two years of experience and a current state license are required.

Qualifications

• Graduate of an accredited physical therapy program.
• At least 2 years' experience as a physical therapist.
• Current Physical Therapist state license.

Responsibilities

• Performs and documents an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plans, prepares, or carries out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Records prognosis, treatment, response, and progress in patient's chart or enters information into computer.
• Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit.
• Confers with the patient, medical practitioners, or other appropriate parties to plan, implement, or assess the intervention program.
• Complies with professional, ethical, and legal standards.
