Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Central Ohio. You will evaluate, develop, and implement individualized treatment plans that improve mobility, restore function, reduce pain, and promote independence for residents in assisted and independent living communities.

Qualifications

• Current State Physical Therapist (PT) license.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
