Origin Travel Nurses seeks a Physical Therapist for home health in Missoula, MT. You will evaluate patients, develop a care plan, and deliver skilled therapy in the home setting under physician orders.

The role includes coordinating with disciplines, supervising PT Assistants, and maintaining CMS-compliant documentation and licensure. Typical schedule is weekday with occasional evenings or weekends during the 13-week contract.

Qualifications

• BS degree in Physical Therapy from an accredited program.
• Current Montana PT license and valid driver's license.
• Two years of PT experience preferred.

Responsibilities

• Evaluate and establish a home health plan of care.
• Coordinate care with other disciplines and obtain orders as needed.
• Provide direct patient care per plan and physician orders.
• Apply evidence-based practice guidelines and Medicare requirements.
• Modify treatment plans as appropriate, including frequency and duration.
• Complete CMS OASIS scoring with the Quality team.
• Participate in multidisciplinary visits to plan care.
• Document services, adverse events, and communications in the medical record.
• Maintain productivity per agency expectations.
• Oversee PT Aide supervision and visits.
• Maintain licensure and continuing education requirements.

Typically M-F 8:00am-5:00pm, with occasional evening/weekend coverage.

Job Description

The Physical Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education.

Responsibilities

• Evaluate and establish a plan of care to meet patients' needs in the home environment.
• Coordinate care with other disciplines and seek orders for appropriate additional services.
• Provide direct patient care based on established care plans and physician orders.
• Apply knowledge of physical therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation.
• Modify treatment plans as appropriate, including frequency and duration of care.
• Master scoring of the CMS required OASIS and collaborate with the Quality team.
• Participate in multidisciplinary conferences to establish relevant care plans.
• Document all skilled services provided, adverse events, and relevant communications in the medical record.
• Manage productivity standards in line with agency expectations.
• Oversee services provided by the Physical Therapist Assistant and perform supervisory visits.
• Maintain requirements of physical therapy licensure, including continuing education standards.

Required Experience / Certifications / Licensure

• BS degree or better from an accredited Physical Therapy program.
• Current license to practice in the State of Montana.
• Valid driver’s license, reliable transportation, and appropriate auto insurance.
• Two years of previous PT experience preferred.
