Job summary

ARMStaffing is seeking a Home Health Physical Therapist to deliver skilled PT services to home health and hospice patients in Lewistown, MT. You will perform comprehensive evaluations, establish treatment plans, and guide patients through therapeutic exercises to improve mobility and independence.

Travel to patients’ homes, document progress, and educate patients and caregivers on home exercise programs. Montana PT licensure, a valid driver’s license, and BLS are required.

• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Travel to patient homes and various settings to provide therapy services.

Job Description

The Home Health Physical Therapist provides skilled physical therapy interventions to Home Health and Hospice patients. This role involves comprehensive patient evaluations, establishing treatment plans, and delivering therapeutic exercises and mobility training to promote independence and improve functional status.

Responsibilities

• Review physician therapy orders and complete comprehensive patient evaluations.
• Establish written treatment plans with short-term and long-term goals.
• Provide therapeutic exercises, stretching, mobility training, and pain management.

Required Experience / Certifications / Licensure

• Current Physical Therapist licensure in the state of Montana.
• Valid driver's license and BLS certification.

Schedule

This position is a day shift, requiring 36 hours of work per week.
