Job summary

Option Care Health is seeking a Clinical Pharmacist – PT, PD, CA to provide safe and effective pharmacy services to home care patients. You will collaborate with Nursing and other professionals to educate patients and caregivers and coordinate care.

This entry-level position requires a PharmD, active state license and eligible IV/compounding experience, with travel up to 5%. A supportive team and a comprehensive benefits program are offered.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license in the relevant state.
• One of: sterile IV admixture; non-sterile compounding/immunizations/MTM; ASHP residency; APPE; or equivalent.

Responsibilities

• Assess home care suitability and collect patient-specific information.
• Design and monitor patient-specific pharmacy care plans for home care patients.
• Provide medication-use education to patients, caregivers and staff.
• Ensure compliance with safety, regulatory, and inventory requirements.
