Job summary
Option Care Health is seeking an entry-level Clinical Pharmacist – PT for home infusion services. The role collaborates with Nursing to coordinate patient care, educate patients and caregivers, and ensure safe, effective pharmacotherapy in a home setting.
The position requires Pharmacy licensure, a Doctor of Pharmacy degree, and training in MTM or related residency programs. Travel within state may be required, with a comprehensive benefits package offered.
Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- ASHP-accredited Pharmacy Residency Program or equivalent experience preferred.
Responsibilities
- Determines patient suitability for home care and collects necessary information.
- Develops and monitors patient-specific pharmacy care plans for home care.
- Modifies care plans based on monitoring data and lab values.
- Provides medication-use education to staff, patients and caregivers.
- Ensures continuity of pharmaceutical care across home and other settings.
- Handles medication preparation ensuring safety, accuracy and compliance with regulations.
- Assists in managing equipment used in medication administration and reporting incidents.
