Responsibilities

• Perform patient evaluations by obtaining history, performing relevant systems review, and selecting tests and measures.
• Establish individualized plans of care addressing impairments per physician orders.
• Conduct ongoing reassessment of patient progress.
• Initiate and manage discharge planning from the first visit through treatment.
• Supervise and delegate to ancillary staff and students per state laws and policy.
• Complete legible, accurate documentation within department policy and accreditation requirements.
• Communicate with patient, family, caregivers, and health care team to maximize benefit using AIDET.
• Educate patient, family, caregiver, or significant other to meet their needs.
• Participate in professional development and educational programs.
• Contribute to quality improvement and safety initiatives.
