Job summary

Nightingale Home Healthcare is seeking a dedicated Physical Therapist to join our growing team in the Norwalk/Stamford area. You will evaluate patients, initiate therapy within 72 hours of referral, and develop individualized plans of care for home health patients.

Qualifications

• Current Physical Therapist license in Connecticut.
• Minimum of 1 year PT experience; Home Health and OASIS experience preferred.
• Experience with WellSky (Kinnser) software preferred.
• Active TB test within 12 months and CPR certification.

Responsibilities

• Evaluate patients and initiate therapy within 72 hours of referral.
• Develop and implement individualized plans of care.
• Supervise PTAs and/or HHAs as assigned.
• Coordinate care with physicians and caregivers within the rehab team.
• Maintain accurate documentation and timely visit notes.
• Conduct supervisory visits and manage discharge planning.
• Provide in-service education and participate in team meetings.
