Qualifications

• Current Physical Therapist license in Connecticut.
• Minimum of 1 year PT experience; Home Health and OASIS experience preferred.
• Valid TB test (within 12 months) and current CPR certification.

Responsibilities

• Evaluate patients and initiate therapy within 72 hours of referral.
• Develop and implement individualized Plans of Care.
• Supervise PT Assistants and coordinate care.

Skills

Current Physical Therapist license in Connecticut

Minimum of 1 year PT experience

Home Health experience preferred

Experience with WellSky (Kinnser) software
