Job summary

Nightingale Home Healthcare is seeking a Licensed Physical Therapist to deliver home-based PT services in Taunton, MA. This part-time role involves evaluating patients, creating and implementing individualized plans, and coordinating with the care team to ensure safe, effective therapy at home.

The position offers per-visit pay, mileage reimbursement, and opportunities for merit-based raises, with 3 weeks of paid time off and a flexible schedule to fit your life.

Qualifications

• Licensed Physical Therapist (PT) in good standing.
• CPR certification current.
• OASIS/WellSky experience preferred (training provided).
• Valid driver’s license and current TB test.
• Orientation at Taunton, MA office required.

Responsibilities

• Evaluate patients’ physical conditions, limitations, and rehabilitation needs.
• Develop and implement individualized treatment plans.
• Provide therapy per physician’s Plan of Care.
• Supervise and coordinate care with PTAs and HHAs.
• Maintain accurate documentation and progress notes.
• Educate patients and caregivers on exercises and equipment use.
• Assess home environments and recommend modifications.
• Collaborate with nurses, social workers and other providers.
• Submit visit notes on time in HIPAA-compliant manner.
