  • Assist in evaluating direct care staff performance.
  • Maintain inventory of office supplies.
  • Cover responsibilities of the Program Administrator when needed.
  • Perform related duties as assigned.

JOB DESCRIPTION

Job Title:Home Care Scheduler

Program:Home Care Services

Reports to:Program Administrator

FLSA:Non- Exempt

Primary Responsibility

Responsible for the implementation and maintenance of service delivery in the greater Leominster program area.

  • Serve as a liaison between client and family, case managers and Aging Services Access Point (ASAP)
  • Process phone referrals and inquiries.
  • Prepare weekly scheduling tasks and activities of direct care staff.
  • Monitor delivery consistent with authorized services.
  • Assist in the evaluation of direct care staff.
  • Maintain inventory of office supplies.
  • Cover the absence of the Program Administrator.
  • Perform assigned duties and tasks of a similar nature.

Licenses/Certifications

  • Current, valid driver’s license and access to reliable transportation required.

Knowledge/Experience

  • Preferred Experience working in a home care setting and familiarity with industry practices.
  • Sensitive to the needs of the aged, handicapped, families and children.
  • Knowledge of standard office procedures, practices, and administrative functions.

Skills/Abilities

  • Ability to read, write, and communicate effectively in English.
  • Ability to prepare accurate, professional, and legible reports and documentation.
  • Proficiency in Microsoft Office applications, including Word and Outlook.
  • Ability to exercise sound judgment and make appropriate decisions in a variety of situations.
  • Ability to assess and respond to client needs and effectively implement and follow established care plans.
  • Strong interpersonal and communication skills, with the ability to work effectively with staff, clients, families, and community-based social service agencies.
  • Ability to remain flexible, responsive, and supportive of the needs of others while maintaining reliability and effectively responding to emergency situations.
  • Ability to work effectively and collaboratively in a team environment.
  • Ability to understand and follow written and verbal instructions.
  • Strong organizational skills, initiative, and the ability to recognize and work within the appropriate scope of responsibility.
  • Ability to work 11:00 AM - 4:00 PM every other holiday as the position requires.

Physical/Mental Requirements

  • Ability to climb stairs to access all areas of Catholic Charities Worcester County facilities.
  • Normal range of hearing and vision, with or without correction.
  • Ability to sit, stand, walk, bend, lift, reach, and perform routine office and program-related activities.
  • Ability to always maintain confidentiality and professional boundaries.
