Job summary

IHA in Mishawaka, IN is seeking a Full-Time Physical Therapist to join our collaborative therapy team. You will assess, treat, and guide patients through personalized recovery plans in a supportive, multidisciplinary environment.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in Indiana.
• Current CPR certification.

Responsibilities

• Assess and treat patients referred to the Physical Therapy department.
• Develop and implement individualized treatment plans based on evaluations and patient goals.
• Educate patients and families on diagnoses, therapy plans, and home exercises.
• Monitor progress, adjust treatment plans, and prepare discharge recommendations.
• Document evaluations, progress notes, and discharge summaries accurately.
• Collaborate with interdisciplinary teams and participate in staff meetings and case management.
• Supervise Physical Therapy Assistants, rehab techs, and volunteers.
• Participate in departmental initiatives and student programs.
