Job summary

Hallmark Manor (WA) is seeking a Physical Therapist to provide direct rehab care to patients upon physician referral in a Life Care setting. You will work within an interdisciplinary team to promote functional progress and quality of life in a skilled nursing environment.

Applicants must be a graduate of an accredited PT program and currently licensed in Washington, with CPR certification. New grads welcome; sign-on bonus available and full-time benefits apply.

Qualifications

• Graduate of an accredited physical therapy program with current state license.
• Active license in the appropriate state and maintain it in good standing.
• CPR certification required or obtained on orientation.

Responsibilities

• Follow established physical therapy treatment plans for patients.
• Assess and adjust goals that are realistic, measurable, and functional.
• Oversee care provided by PTAs and rehab staff; supervise students as needed.
• Document care and progress in therapy records; use therapy software.
• Provide excellent customer service and collaborate on an interdisciplinary team.
• Maintain patient safety, body mechanics, and attendance.
