Job summary

Evolve Therapy Services is seeking a licensed Physical Therapy Assistant to join our in-house Skilled Nursing Facility partner in Freeport, IL. Under the supervision of a Physical Therapist, you will deliver patient-centered therapy to residents and help implement treatment plans.

Responsibilities include screenings, progress notes in NetHealth EMR, scheduling with the care team, and contributing to quality resident care.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/tatient/treatment.
• Assist the Physical Therapist in identifying treatment goals and plans.
• Conduct individual and group treatment as outlined in the treatment plan.
• Maintain compliant and medically necessary documentation in NetHealth EMR.
