Job summary

Evolve Therapy Services, LLC in Freeport, IL is seeking a licensed Physical Therapy Assistant to join our in-house Skilled Nursing Facility team. Under the supervision of a Physical Therapist, you will help plan, deliver, and document therapy services to residents.

Qualifications

• Requires an Associate degree in Physical Therapy and ongoing continuing education.
• Active Physical Therapy Assistant license in state of practice.
• Familiarity with CMS expectations and basic documentation in EMR.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine intervention needs.
• Assist in evaluations and development of target goals and treatment plans.
• Conduct individual and group treatments addressing strength, mobility, gait, ROM, and balance.
• Assist in selecting techniques/media and adapting activities per resident performance.
• Monitor residents’ responses and consult the Physical Therapist for modifications.
• Assist in scheduling and coordinating treatment with the team.
• Train residents, families, and staff in equipment use and safety.
• Maintain timely, compliant documentation and participate in QA activities.
