Telos Health Systems is seeking a Psychotherapist licensed LMHC, LCSW, LMFT, or as a licensed psychologist (PsyD/Ph.D.) to provide mental health evaluations and psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities in Palm Bay, FL and Stuart, FL. Qualifications

Schedule: Pay Per Encounter | Fee-for-Service

Employment Type: W-2 - Part Time - 2 days per week (non consecutive days) - you pick the days out of Monday - Friday

Job Summary

Telos Health Systems is seeking a Psychotherapist (LMHC, LCSW, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

Key Responsibilities

Conduct comprehensive mental health assessments and diagnostic evaluations.

Provide individual psychotherapy and ongoing treatment services.

Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.

Maintain accurate, timely, and compliant clinical documentation.

Coordinate care with facility staff, medical providers, and other healthcare professionals.

Adhere to all federal, state, and organizational standards, policies, and regulations.

Qualifications

Required

Active Florida license as one of the following:

Licensed Mental Health Counselor (LMHC)

Licensed Marriage and Family Therapist (LMFT)

Licensed Psychologist -PsyD/Ph.D.

Master’s degree (LMHC/LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.

Ability to work independently while collaborating effectively within a clinical team.

Strong communication and clinical documentation skills.

Experience working with geriatric populations.

Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.
