St. Catherine’s Center for Children in Albany, NY is seeking a Skill Builder, Psycho-Social Rehabilitation - CFTSS to support youth identified with mental health or developmental challenges in home and community settings.

The role emphasizes collaboration, documentation, and flexible hours, with requirements including a relevant degree or equivalent experience, a valid NYS driver’s license, and proficiency with Microsoft Office and Google Workspace.

Qualifications

• Bachelor's degree in social work, psychology, or related field.
• HS diploma with 2 years' relevant experience in children's mental health.
• Ability to work with diverse youth and families.

Responsibilities

• Engage directly with youth and families in home and community to provide teaching and coaching.
• Deliver services per the Treatment/Service Plan following CFTSS regulations.
• Maintain timely documentation in the electronic health record.
• Local travel to provide services and support to families.
• Collaborate with internal staff and external providers to coordinate care.
• Uphold HIPAA and ensure confidentiality.
