DMC Detroit Receiving Hospital, Michigan’s first Level I Trauma Center, helped pioneer the evolution of emergency medicine and currently has one of the busiest and most well-equipped emergency departments in the region. It houses the state’s first and largest verified burn center, one of only 43 in the nation. The hospital also offers the state’s leading 24/7 hyperbaric oxygen program, Metro Detroit’s first certified primary stroke center, and the nationally recognized DMC Rosa Parks Geriatric Center of Excellence.
Under general direction and according to established policies and procedures, the social worker assesses and attends to the psychosocial needs of patients and their families. Responsibilities include psychosocial evaluation, crisis intervention, psychotherapeutic support and counseling, and psychotherapy. Acts as an advocate to assist patients in their physical, emotional, and social adjustment, facilitating an individualized care plan within a multidisciplinary team. Maintains continuity of care through discharge planning, collaborating with patients, families, guardians, caseworkers, and community resources. Provides ongoing evaluations, monitoring, and counseling, and facilitates adjustment to secondary effects of illness. Acts as a liaison between the facility, family, government, and community agencies. Documents interventions and completes required paperwork, including discharge summaries and admission forms. Responds to crisis situations in accordance with hospital policies. Maintains current referral sources and uses them appropriately. Performs other duties as assigned.
