Responsibilities

• Assess adolescents, adults, and geriatric patients and refer as needed.
• Communicate effectively both verbally and in writing.
• Coordinate referrals to internal and external resources.
• Travel to other sites within the health system for assessments when needed.
• Note: the role does not provide direct patient care.
